We help residents and families navigate important legal matters related to care and planning, including guidance on advance directives, power of attorney, and access to community legal resources. 

Mental Health Services 

Our social workers provide support, counseling, and referrals to address depression, anxiety, adjustment challenges, and other mental health needs to promote coping, engagement, and a positive quality of life during their stay. 

Discharge Planning & Transitions

We coordinate services and connect with our community resources so each resident has a personalized plan that supports continued health and independence. During your stay at Evan Terrace Post Acute, we provide transportation to any off-site medical appointments you may have.
