Who should I call to get started?

Contact us by phone or email to discuss with us or schedule a video chat or tour. We’ll be happy to answer all of your questions, introduce you to our staff, and get to know you a little bit better. 

What articles of clothing should we bring?

We recommend you to bring a pair of comfortable shoes and at least seven days worth of comfortable clothing including pajamas and undergarments. We encourage you to bring personal items such as family photos, blankets, and decorative items. Anything that will help make your stay more comfortable. 

Can I bring personal belongings?

Yes. Residents are encouraged to bring family photos, blankets, decorations, books, and other personal items to help make their room feel comfortable and familiar. Personal belongings should be clearly labeled whenever possible. 

Insurance & Billing

What insurances do you accept?

We proudly accept a wide variety of insurance options, Medicare Medicaid, most managed Medicare/medicaid plans, private pay, and more. We do not currently accept VA ICF/Long Term or Kaiser Permanente. Whatever insurance you have, our insurance experts will take time to help you understand your plan’s benefits. 

Who do I talk to about questions in regard to financials, billing, and/or admissions paperwork?

We employ a full-time Business Office Manager for all financial and billing concerns. Should you have any questions regarding these matters please make an appointment to see our Business Office Manager by contacting the front desk/receptionist. 

Our Admissions Coordinator will guide you through the required forms on admission and review the rules and regulations regarding Medicare, Medicaid, and various insurances.

Does Evan Terrace Post Acute offer rehabilitation?

Yes. At Evan Terrace Post Acute, your successful recovery and quick return home is our top priority. We provide a unique personalized experience whether you’re suffering from an illness or recovering from a major medical event. Physical, occupational, and speech therapy is conducted under the guidance of licensed experts and is available six days a week. 

When will the doctor see me?

Physicians typically visit within 24 hours of admission. Your nurse will communicate any medical concerns with your doctor before and after they see you to ensure that you are properly accommodated and that we can support you fully. 

How often will I have therapy?

Most patients, depending on their condition and when they are admitted, will start rehab within 24 hours of admission and will receive therapy up to six times per week. This can change depending on assessments, personal preferences/needs, and other limiting health factors. 

Do I need to bring my medications?

No, you do not need to bring your own medications unless specifically told to do so. Upon admission, we will order your medications using our off-site, local pharmacy, then securely store them on-site to ensure safety. 

How long is a typical stay?

Each case is different and unique. Patient progress is reviewed weekly by our interdisciplinary team of nurses, therapists, and dietitians. Our main goal is for you to reach your highest level of independence as safely and efficiently as possible, and we will update you on the outlook of the duration of your stay as your care progresses.

We offer daily activities suited to a wide variety of interests. For your convenience, there will be an activity calendar posted in your room. Please contact the Activities Director for additional information about our daily activities schedule. 

What is your dining program like?

At Evan Terrace Post Acute, we’re proud to offer restaurant-style dining with diets designed by our full-time, in-house Registered Dietitian. Every meal will be carefully crafted to support your healing without sacrificing flavor. 

What role does Social Services play?

Our social services team assists both residents and their families during transitional periods. They also provide essential information, manage requests and concerns, and help in care and discharge planning for each resident. They can arrange professional services for Dental, Vision, Podiatry, Audiology, Power of Attorney, Dementia assessment, Psychosocial and Psychiatric Evaluation, and individual therapy. 

Policies

Are pets allowed?

We are a pet friendly skilled nursing facility. We require your pet to be up-to-date on vaccinations for the safety and comfort of our other residents. 

What is your visitor policy?

At Evan Terrace Post Acute, we believe that family and friends play a critical role in our residents’ emotional and physical well-being. Contact us by phone for the most current, detailed information regarding available visitation options. 

Can I bring in my loved one’s favorite food?

Consistent with the provision of special diets, always consult with the nursing staff before bringing food to residents. Any food kept in the resident room must be in properly sealed containers.

Is smoking allowed?

We have a separate, designated covered smoking area for the patients of ours who smoke.

Questions & Concerns

How do you protect patients, staff, and visitors from COVID-19 and other infectious diseases?

Our team is highly trained and experienced in caring for high-acuity patients and infectious diseases like COVID-19. Our dedicated infection prevention nurse helps ensure we follow current infection prevention guidelines and best practices. This includes educating all staff about the appropriate use of personal protective equipment (PPE) such as N95 masks, surgical masks, gloves, and gowns and when to use COVID-19 protocols required by local department of public health and/or the CDC. 

What do we do if we have personnel concerns?

Our primary concern is that your loved one feels comfortable here and that their needs are being met. You should feel secure and at ease with our staff, particularly those providing direct care. If you have any concerns or praise for our staff, please feel free to contact our social services department. They will be able to address any apprehensions you may have regarding any particular staff members.
